File a Claim
IMPORTANT: The Settlement Administrator may request supporting purchase documentation in addition to the documentation and information requested below, so please retain all purchase records. The Settlement Administrator may reject a claim if the Class Member or their authorized agent does not provide all requested documentation in a timely manner.
Please be advised, it is the responsibility of the claimant to provide and maintain accurate, complete, and up-to-date contact information throughout the claims process. Failure to update such information may result in the denial of the claim.
BEFORE FILING AN ONLINE CLAIM, PLEASE READ THESE INSTRUCTIONS:
There are several Firewall factors that could affect your ability to file a claim online, and we recommend the following:
- Turn off the pop-up blocker in the browser settings.
- Do not use any autofill options.
- File your claim using “incognito mode” – this will disable any browser extensions that could be blocking the pop-ups.
- Try using a different browser, such as Chrome, Firefox, etc. (Safari seems to cause issues).
- If you encounter problems, try filing on a different device.
- When entering the amount you paid, you need to enter the amount in dollars and cents with a decimal point, but no dollar sign. (For example, 100.41)
If you still experience issues filing a claim online, you may either download and print a blank claim form by following the link on this page, complete it, and mail it to the address provided on the website or submit via email to info@ChantixSettlement.com.
Individual Consumers
If you are a member of the Class as a consumer, you may file a claim for a pro rata share of the Settlement Fund. You must complete a Claim Form and mail it to the Settlement Administrator, or submit your claim online by September 14, 2026, through the link below.

Third-Party Payors (TPPs)
Third Party Payors (TPPs) are entities (besides the patient) that provide payment or reimbursement for health care expenses, like prescription drug benefits. They include entities such as, self-insured health and welfare plans and health insurance companies that make payments from their own funds, and other health benefit providers and entities with self-funded plans that contract with a pharmacy benefit manager (PBM) or administrator to administer their prescription drug benefits for their members, employees, insureds, participants, or beneficiaries.
TPP Claim Forms must be postmarked (if mailed) or received (if submitted online) no later than September 14, 2026.
